ABSTRACT

Utilizadas de forma complementaria, alternada o integrada con la medicina convencional, la demanda de la población por las terapias no convencionales se ha incrementado considerablemente en las últimas décadas, requiriendo de los médicos un conocimiento de las nociones básicas de tales enfoques terapéuticos para orientar a sus pacientes en relación con tratamientos distintos a aquellos que suelen prescribir. Entre las mismas, la homeopatía y la acupuntura se han considerado especialidades médicas en Brasil durante varias décadas. Objetivo: Describir el estado actual de la formación médica en terapias no convencionales (homeopatía y acupuntura) alrededor del mundo. Métodos: Actualizamos los datos relacionados con los estudios y revisiones publicados hasta 2013 a través de una revisión de los estudios más recientes incluidos en la base de datos PubMed. Resultados: En todos los países, la enseñanza de terapias no convencionales se considera un tema relevante para la formación de médicos como una función del creciente interés de la población en su uso, con un amplia variedad de enfoques dirigidos a los estudiantes de licenciatura y posgrado, médicos residentes y profesionales con otras especialidades médicas. Conclusiones: Las escuelas brasileñas de medicina deben proporcionar a los estudiantes de licenciatura y posgrado, así como a los médicos residentes, un conocimiento preciso de los supuestos teóricos y enfoques clínico-terapéuticos propios de la homeopatía y la acupuntura, entre otras terapias no convencionales.


Used as complementary to, alternating or integrated with mainstream medicine, the population's demand for non-conventional therapies has substantially increased in the past decades, requiring from doctors knowledge on the basic notions of such therapeutics to orient their patients regarding treatments different to the ones they usually prescribe. Among them, homeopathy and acupuncture are considered medical specialties in Brazil for various decades. Aim: To describe the current state of medical education in non-conventional therapies (homeopathy and acupuncture) around the world. Methods: We updated data resulting from studies and reviews published until 2013 through a review of more recent studies included in database PubMed. Results: In all countries the teaching of non-conventional therapies is considered a relevant topic for the training of doctors as a function of the increasing interest of the population in their use, with a broad range of approaches targeting undergraduate and graduate students, medical residents and doctors from other medical specialties. Conclusions: The Brazilian medical schools must provide undergraduate and graduate students and medical residents accurate knowledge on the theoretical assumptions and clinical-therapeutic approaches proper to homeopathy and acupuncture, among other non-conventional therapies.

ABSTRACT

Semiology, Clinical Pathophysiology, Phytotherapy and Homeopathy were taught through active methodology by the Moodle platform (HL) also using apps for monitoring injuries through SAPB site, building rationality for the external topic use of homeopathic and herbal medicines.At the 7th Infirmary of General Hospital Santa Casa de Misericordia in Rio de Janeiro, the clinical medical team of the Benoit Mure Nucleus of Assistance Teaching and Research,made a medicinal plant gardenwith the agricultural/environmental educator, and communityethnicalleaders. Undergraduate students from UNIFASE/FMP Academic League of Physiology and Pathophysiology plantedCalendula Officinalis seedlings, destined to research in pharmacobotany and to be in touch with pharmacotechnical prepares. Aims: To follow-up the study since from seedling of Calendula Officinalistill to prepare of HUD. Methodology: To prepare medicinal garden: plant seedlings of Calendula offin accordance with good agricultural techniques. At first: Three seeds in ten pots, made in triplicate (N = 90), to obtain explants of quality analyzed whose evaluation was about plant development (explants percentual: weight, size, viability, morphology). To visit pharmacy-school to learn how to preparematrix extracts and High Ultra-Diluted Solutions (HUD) of Calendula off 6DH. Results and Discussion: The stages of teaching semiology and practice in pharmacobotany, besides cultive plant have been completed.Viability was impaired because of lack of irrigation during 4 months (50%). Weight, sizewere minor than usual but with no statistical significance (p>0.05). Morphology was maintained. Unfortunately, the visit to pharmacy-school couldn't be performed at this period. Conclusion: Through hybrid learning and face-to-face activities completely realized, students had qualitative perception of the relevance about morphology and quality of plant development, chemistry plant production, aiming for resolutive actions based upon pathophysiology of chronic diseases. Because of pandemic, it was described just a preliminary experience. After returning to normal activities the experiment will be repeated.

ABSTRACT

Undergraduate students of UNIFASE/FMP, through the Academic League of Human Physiology and Pathophysiology (Liga Acadêmica de Fisiologia e FisiopatologiaHumana -LAFFH), participate in a supervised internship to outpatient at the Infectious and Parasitic DiseasesService of FMP/FASE, authorized by Clinical Direction (October 2018). Patients received conventional therapy in addition to complementary therapy with Homeopathy, for external use for cleaning skin lesions and modulating inflammatory responses, whose mechanisms of action are already described in the scientific literature and authorized by ANVISA. Aims: Make approach of undergraduate students to Homeopathic pathophysiology. Methodology: The supervised clinical internship of LAFFH students is in two service rooms with 6 students in each, based on prior appointment due to spontaneous demand, with medical records registered by the IPD outpatient service secretary. Outpatient clinical care addresses the pathophysiology and miasms of the symptoms and signs of neglected chronic parasitic diseases prevalent in our country, identified by anamnesis and physical examination. They are correlated to biological activities in plant chemistry classes, besides official homeopathic pharmacopoeia. So, it was adopted the external use of HUD Ledum palustre 6ch for the cleansing of skin lesions with torpid evolution at IPD. Results and discussion: Two patients with edemaand varicose ulcers were treated with the ointment offered. Edema extension which was +++/4+ to both patients diminished to +/4+ in 30 days of interval. Indeed, ulcers remained at the same shape and size, but their bases were cleaned, without purulence. Novel clinical sessions did not occur because of social distancing needed. Conclusion: It has been developed a positive perception among patients and students about the action of homeopathic remedies. The practical teaching activity initiated encourages students in clinical reasoning about complementary therapeutic association with Homeopathy for patients whose evolution is below the expected prognosis with conventional therapy. Project of clinical observation will be elaborated to research ethics committee.

ABSTRACT

La formación de recursos humanos de elevada cualificación en el área homeopática debe encararse como una de las prioridades para una sólida y definitiva consolidación de la Homeopatía en territorio brasileño. La cuestión de la estructuración de la Homeopatía en Brasil, por su parte, no puede ser convenientemente entendida sin un análisis de los sistemas formadores y captadores de recursos humanos homeopáticos. Es decir, el análisis debe recaer sobre la enseñanza homeopática de graduación y posgrado, pero también sobre las condiciones del mercado de trabajo para el médico homeópata. La reflexión sobre la enseñanza de la Homeopatía, en cambio, conduce inevitablemente al abordaje del problema de la investigación homeopática. Este artículo no tiene la intención de realizar un diagnóstico exhaustivo de la situación de la Homeopatía en lo que se refiere a la enseñanza, la investigación y el mercado de trabajo en Brasil. Sin embargo, a partir de algunos presupuestos y constataciones personales, delinearemos un marco general que permita definir algunas directrices relativas al problema de estructuración de la Homeopatía en Brasil.


The formation of highly qualified human resources in the homeopathic sector must be faced as a priority for a solid and definitive consolidation of Homeopathy in Brazilian territory. On the other hand, the structuring of Homeopathy in Brazil, cannot be conveniently understood without an analysis of both the training and recruiting systems for homeopathic human resources. In other words, the analysis must not only fall upon homeopathic teaching at a postgraduate level, but also the job-market conditions for the homeopathic physician. On the other hand, the reflection upon the teaching of Homeopathy, inevitably leads to an approach to the problem of homeopathic investigation. This article does not have the intention of carrying out an exhaustive diagnosis of the educational, investigational and jobmarket aspects of Homeopathy in Brazil. However, on some based budgets and personnel records, a general framework that will allow the definition of several directives pertaining to the problem of the structuration of Homeopathy in Brazil, will be outlined.

Resumo: Introdução: Currículos médicos de graduação são extremamente carregados em seu conteúdo, tornando-se primordial a necessidade de otimizar competências essenciais. As Diretrizes Curriculares Nacionais (DCN) do Curso de Graduação em Medicina preconizam uma valorização do tratamento do doente que possui muitas interfaces com a homeopatia. Apesar de ser uma especialidade médica no Brasil reconhecida desde 1980, a homeopatia ainda é pouco presente na graduação médica. Objetivo: Este estudo teve como objetivo desenvolver uma matriz de competências essenciais composta de conhecimentos e habilidades desejáveis para o ensino da homeopatia na graduação médica. Métodos: Trata-se de um estudo quanti-qualitativo realizado por meio da técnica Delphi normativa, em duas rodadas, com os docentes especialistas em homeopatia do país. Na primeira rodada, aplicou-se um questionário anônimo on-line para identificar as competências (conhecimentos e habilidades) necessárias em homeopatia para os discentes de graduação em Medicina. Após a análise de conteúdo, os temas foram agrupados nessas duas categorias que retornaram para apuração na segunda rodada, em que os especialistas assinalaram o grau de concordância (escala de Likert de quatro pontos: de não relevante a muito relevante). Para a definição de consenso, adotaram-se dois critérios: índice de De Loe e uma nota de relevância adotada pelos autores com vistas a discriminar mais os graus de consenso. Resultados: Dos 14 temas/subtemas relacionados como competências/conhecimentos, consideraram-se 11 como de alto consenso, dos quais cinco foram avaliados como primordiais, pois obtiveram nota de relevância acima de 9,0 (três relacionados ao grande tema clínica homeopática; um, à teoria e técnica homeopáticas; e outro, à pesquisa homeopática). Em relação às competências/habilidades, oito das 11 foram consideradas de alto consenso, e somente duas alcançaram nota acima de 9,0, ambas relacionadas à clínica homeopática. Conclusão: O ensino da homeopatia pode contribuir para a mudança paradigmática da medicina no sentido de valorizar o doente e promover a saúde, permitindo uma atuação médica mais humanizada e centrada no doente, o que justificaria a adoção de seu ensino, inclusive obrigatório, nas faculdades de Medicina. A elaboração de uma matriz de competências do que deve ser ensinado de homeopatia aos discentes da graduação médica vem ao encontro das DCN e instrumentaliza a reflexão na elaboração de uma futura ementa.


Abstract: Introduction: Undergraduate medical curricula are overloaded with content, making the need to optimize essential skills and knowledge paramount. The National Curricular Guidelines for the medical course (DCNs) put focus on patient treatment, which involves several interfaces with homeopathy. However, despite it being recognized as a medical specialty in Brazil since 1980, homeopathy is rarely present in undergraduate medical education. Objective: To develop a collection of essential skills and knowledge for teaching homeopathy as part of the undergraduate medical curriculum. Methods: Quanti-qualitative study performed using the standard Delphi technique in two rounds with specialized homeopathy teachers from Brazil. An anonymous online questionnaire was conducted to identify the learning competences needed in homeopathy for undergraduate medical students. Following content analysis, the themes and sub-themes were grouped into the two main categories of knowledge and skills, which were returned to the specialists who rated their importance for each item using the "Four-Point Likert Scale" from not at all important to very important. For the purpose of generating a general consensus, two criteria were adopted: the "De Loe index" and a importance score. Results: There was a high degree of consensus regarding eleven of the fourteen themes/sub-themes, five of which were considered essential, with an importance score above 9.0 (three related to the general theme, Homeopathic Clinic, one to Homeopathic Theory and Method and another to Homeopathic Research). There was also a high level of consensus regarding eight of the eleven skills, but only two gained a score of above 9.0, both related to Homeopathic Clinic. Conclusion: The teaching of Homeopathy can contribute to a paradigmatic change in medicine, particularly in the sense of prioritizing the patient and promoting health, as well as allowing for a more humanized and patient-centered medical engagement. These factors would justify the adoption of its teaching, so much so it could be made mandatory across all medical study. The establishment of a "competency matrix" for homeopathy study, which ought to be taught to students of the medical field, meets the DCNs and would be instrumental in future syllabuses.

ABSTRACT

Homeopathic education has passed through significant incremental changes in the past few years, where especially postgraduate education has become increasingly slanted toward advanced knowledge of clinical work and research methods. Among many educational activities, a great source of learning is from presenting at or attending a journal club meeting, which is a gathering of people to learn and to critically appraise a journal article or other study material. There has been little previous guidance in homeopathy regarding how to prepare and present journal club content. Selection of a suitable topic is one of the critical prerequisites. Each and every step, from preparation to presentation, needs to be carefully planned and considered. For the meeting to be successful, the final discussion phase requires the active participation and critical insight of all those attending.

This article analyzes the practice and professional status of Colombian homeopaths in the twentieth century, based on applications for licenses in the "Teguas" series in the Archivo General de la Nación. Within the historical context of the practice of medicine in Colombia, it studies homeopathic practice within the framework of the debate between licensed and permitted medicine. In that context, the field of homeopathy was subordinate to university medicine and homeopaths were a group of practitioners who were neither homogeneous nor organized, but characterized by their shared struggle to become "entitled" to practice, and their advocacy of professional status through constant litigation against official reprimands.


El artículo analiza la práctica y el estatuto profesional de los homeópatas colombianos en el siglo XX, según las solicitudes de licencia de la serie "Teguas" del Archivo General de la Nación. Desde el contexto histórico del ejercicio de la medicina en Colombia, se estudia la práctica homeopática en su inserción en el debate entre medicina diplomada y medicina permitida. Ahí aparece la homeopatía como un campo subordinado a la medicina universitaria y los homeópatas como conjunto de practicantes no homogéneo ni organizado, pero caracterizado por compartir la lucha por el "derecho adquirido" a ejercer y por la defensa de un estatuto profesional mediante la judicialización constante de la reprobación oficial.

Desde el contexto histórico del ejercicio de la medicina en Colombia, se estudia la práctica homeopática en su inserción en el debate entre medicina diplomada y medicina permitida. Ahí aparece la homeopatía como un campo subordinado a la medicina universitaria y los homeópatas como conjunto de practicantes no homogéneo ni organizado, pero caracterizado por compartir la lucha por el "derecho adquirido" a ejercer y por la defensa de un estatuto profesional mediante la judicialización constante de la reprobación oficial.

ABSTRACT

North American naturopathic medicine is a distinct form of practice that is woven into the larger fabric of integrative medicine; in a number of US states and Canadian provinces, naturopathic doctors enjoy a wide scope of practice, including the ability to make diagnoses, order tests, use medical technology, write prescription drugs, and perform minor surgeries. However, the basic premise of naturopathic medicine and its guiding principles-considering the whole person and supporting healthy lifestyle behaviors-is the unifying approach in clinical practice. In the 1970s, homeopathy-considered in many circles to be a hypothesis-driven, fringe form of alternative medicine-became embedded into the training and practice of North American naturopathic doctors. Since the earliest days of its theory (circa 1800), homeopathy has escaped, and continues to escape, biological plausibility; however, the persistence of this modality (and the insistence by both its consumers and practitioners that it provides benefit) speaks to the role of expectations, beliefs, values, agency, context effects, and the placebo-at-large. It is our contention that the progression of professional naturopathic medicine in the 21st century requires a major transition in how it approaches the subject of homeopathy. We propose that students should be encouraged to critically analyze the tenets of homeopathy, its lesser known history, and the idea of homeopathy as a biomedicine that simply awaits untold chemicophysical mechanisms. Furthermore, the modality of homeopathy should be incorporated into the larger context of placebo studies, narrative medicine, ethics, and psychotherapeutic techniques.

ABSTRACT

PURPOSE: To examine whether demographic differences exist in United States Medical Licensing Examination (USMLE) scores and the extent to which any differences are explained by students' prior academic achievement. METHOD: The authors completed hierarchical linear modeling of data for U.S. and Canadian allopathic and osteopathic medical graduates testing on USMLE Step 1 during or after 2010, and completing USMLE Step 3 by 2015. Main outcome measures were computer-based USMLE examinations: Step 1, Step 2 Clinical Knowledge, and Step 3. Test-taker characteristics included sex, self-identified race, U.S. citizenship status, English as a second language, and age at first Step 1 attempt. Covariates included composite Medical College Admission Test (MCAT) scores, undergraduate grade point average (GPA), and previous USMLE scores. RESULTS: A total of 45,154 examinees from 172 medical schools met the inclusion criteria. The sample was 67% white and 48% female; 3.7% non-U.S. citizens; and 7.4% with English as a second language. Hierarchical linear models examined demographic variables with and without covariates including MCAT scores and GPA. All Step examinations showed significant differences by gender after adding covariates, varying by Step. Racial differences were observed for each Step, attenuated by the addition of covariates. CONCLUSIONS: Demographic differences in USMLE performance were tempered by previous examination performance and undergraduate performance. Additional research is required to identify factors that contribute to demographic differences, can aid educators' identification of students who would benefit from assistance preparing for USMLE, and can assist residency program directors in assessing performance measures while meeting diversity goals.

ABSTRACT

A formação de recursos humanos de elevada qualificação na área homeopática deve ser encarada como uma das prioridades para uma sólida e definitiva consolidação da homeopatia em território brasileiro. A questão da estruturação da homeopatia no Brasil, por sua vez não pode ser convenientemente entendida sem uma análise dos sistemas formadores e captadores de recursos humanos homeopáticos. Ou seja, a análise deve recair tanto sobre o ensino homeopático de graduação e pós-graduação quanto sobre as condições do mercado de trabalho para o médico homeopata.
